Screening and Assessment Tools
These are not diagnostic instruments on their own, but they are useful starting points for understanding mental health presentations.
PHQ-9 — Patient Health Questionnaire
The most widely used screening tool for depression. A 9-item questionnaire that takes under three minutes to complete. Available in Urdu.
GAD-7 Generalised Anxiety Disorder Scale
A 7-item tool for screening and measuring anxiety severity. Simple, validated, and widely used alongside the PHQ-9.
PCL-5 PTSD Checklist for DSM-5
A 20-item self-report measure of PTSD symptom severity. Particularly relevant in disaster-affected and conflict-exposed populations
SRQ-20 Self-Reporting Questionnaire
Developed by the WHO specifically for use in low and middle income country settings. Designed for non-specialist use in primary care and community contexts.
Columbia Suicide Severity Rating Scale (C-SSRS)
The gold standard tool for suicide risk assessment. A brief, structured interview available in multiple languages.
